22-23 Volunteer Packet (InformedK12)

Please attach your TB test results to be submitted with the Volunteer Packet.  NOTE:  Volunteers who test negative (for TB) shall thereafter be required to take a tuberculosis test (or assessment) every four years in accordance with Education Code 49406.
